Required
  • Bachelor of Science in Nursing (BSN) from an accredited college or university
  • Active California RN License.
  • Minimum 2 years of Outpatient Nurse Coordinator experience
  • Neurology (Neuro) experience is required, all levels of experience considered
  • BLS certification issued by American Heart Association (AHA)
  • Demonstrated experience in Epic EMR system, including documentation and clinical workflow
  • Proficiency with inbasket management and telephone-based clinical coordination

Responsibilities
  • Provide direct, hands-on patient care in accordance with the California Nursing Practice Act, hospital policies, and evidence-based nursing standards.
  • Perform comprehensive patient assessments, interpret clinical data, and determine appropriate nursing diagnoses.
  • Develop, implement, and evaluate individualized patient care plans in collaboration with patients, families, and interdisciplinary care teams.
  • Coordinate patient care primarily through ambulatory/outpatient workflows, including telephone-based communication and Epic inbasket management.
  • Monitor patient status frequently and respond appropriately to changes in condition, including activation of emergency protocols when necessary.
  • Administer nursing and medical interventions safely while monitoring for effectiveness, side effects, and adverse reactions.
  • Provide patient and family education focused on disease management, treatment plans, medication adherence, and preventive care.
  • Deliver family-centered care by considering cultural, developmental, emotional, and physiological needs of patients and caregivers.
  • Manage and prioritize multiple clinical tasks, ensuring timely response to messages, results, and provider requests within Epic.
  • Utilize Epic and other clinical technologies for documentation, communication, care coordination, and clinical workflow management.
  • Collaborate closely with physicians, providers, and interdisciplinary teams to ensure continuity and coordination of care.
  • Support outpatient clinic operations, including telephone triage, message handling, and coordination of referrals or follow-ups.
  • Maintain patient safety standards and actively participate in quality improvement initiatives and compliance monitoring.
  • Participate in professional development activities, maintaining RN licensure, BLS certification, and current clinical knowledge.
  • Contribute to team effectiveness through communication, conflict resolution, and participation in unit-based or shared governance activities.
  • Serve as a resource to peers through informal guidance, precepting, and sharing clinical expertise when needed.
  • Continuously evaluate personal nursing practice in alignment with professional standards, regulations, and best practices.
