Required
  • High School Diploma or GED
  • Current CA Registered Nurse license
  • Current Basic Life Support (BLS) - American Heart Association (AHA)
  • Minimum 1 - 2 years of RN outpatient, behavioral health, psychiatric, primary care or community health
  • Strong assessment, documentation, and communication skills
  • Ability to triage psychiatric symptoms and escalate appropriately
  • Ability to work collaboratively within a multidisciplinary behavioral health team
  • Self-directed and able to manage responsibilities with minimal oversight
  • Ability to work with adults diagnosed with severe mental illnesses
Preferred
  • Behavioral health or psychiatric nursing experience (highly preferred)
  • Experience with county behavioral health systems or public health 
  • Bachelor of Science in Nursing 
  • Knowledge of EPIC EHR, Clinician's Gateway EHR, and Microsoft Office 
  • Experience providing field-based nursing or home visits

Required Abilities/Skills
  • Strong communication and detail-oriented documentation skills
  • Organization abilities for patient coordination, education, and record-keeping
  • Collaboration skills for working with healthcare providers and community engagement.
Responsibilities/Essential Functions
  • Adults 18 years and older; level 1 mental health patients with sever and persistent mental illness
  • Primarily focused on care coordination being the liaison between the patient and the primary care team. 
  • Daily Duties: Support primary care providers during patient visits 
  • Chart prep - preparing medical records, medication lists
  • new patient intakes 
  • procedures (IE: immunizations, EKGs, PAP smears, I&Ds, joint injections, wound care, ear lavages, point-of-care testing)
  • medication administration - oral, injections, nebulizer  
  • Conducting nursing visits 
  • Triage in-person walk-ins and tele-visits
  • Nursing assessments
  • Follow up on hospital admission/ED visits and specialist visits, lab results, health screenings, and diagnostic imaging
  • Injections, Wound Care, Education, Medication reconciliation while adhering to policy and procedures as outlined by standing orders
  • Managing DME, orders, lab orders, diagnostic tests, referrals, medication refills, prior authorizations, medical records 
  • Coordinating care with multidisciplinary care team (ie: case managers, psychiatric care providers), external care providers (ie: hospitals, specialists), and other patient support (ie: IHSS workers, board and care staff, family members)
  • Supporting the psychiatrists nurse with injections as needed
  • Phlebotomy 
  • Weekly Duties: Attending weekly multidisciplinary team meetings, generating and sending PATH reports 
  • Monthly Duties: Monthly medical meetings, PATH meetings with management teams, manage the emergency kit inventory and monthly check off, managing the 403B pharmacy inventory   About Us
