MINMUM QUALIFICATIONS Education Qualifications
High School Diploma or EquivalentÂ
Licenses and Certifications
LVN - Licensed Vocational Nurse
BLS - Basic Life Support
Experience
Minimum 2-3Â years of experience as as an LVN with Ambulatory or Acute care experience
EPIC preferred
Responsibilities:Â
Accurately obtains clinical information and consults with Stanford faculty for appropriate referral.
Communicates the appropriate referral choice to community physicians.
Coordinates and integrates services within department and other departments by continually collaborating with staff and faculty.
Develops Facilitative relationships with referring physicians’ office staff regarding both patient appointment scheduling and clinical information.
Facilitates appointments for referring physicians’ patients within the hospital.
Proactively provides timely patient information and reports to the referring physician and facilitates communication between faculty and referring physicians.
Serves as a liaison between referring physicians and operations related to patient access and requested clinical information.
Works with Hospital Physicians, management and staff to develop referral algorithms, guidelines and resources to facilitate patient referral to the Hospital.
