Power Personnel is seeking a dedicated Hospital Care Coordination Liaison to serve as a vital link between patients, families, and healthcare teams. Competitive Pay: $55.00-$60.00 per hour
Shift Days: Monday-Friday
Shift Time: 8:00AM-4:30PM
Start Date: ASAP
Department: Patient Services
Location: Palo Alto, CA
Duration: 6 Months

Required Qualifications
  • Bachelor’s degree in Social Work, Healthcare Administration, Psychology, Public Health, or related field
  • Three (3) years of relevant healthcare experience
  • Experience in care coordination, case management support, or utilization management
  • Strong communication and interpersonal skills
  • Ability to coordinate effectively with multiple stakeholders
  • Knowledge of insurance benefits, authorizations, and discharge planning workflows

A Day in the Life
  • Act as a liaison among patients, families, and clinical teams
  • Support patient needs through advocacy and communication
  • Prepare clinical documentation for insurance reviews
  • Assist with discharge planning and post-acute referrals
  • Manage medical records and follow up with providers
  • Support department operations and data entry tasks
