Required
  • High School Diploma or GED
  • Current CA Registered Nurse license
  • BSN Degree 
  • Current Basic Life Support (BLS) - American Heart Association (AHA)
  • Minimum 1 - 2 years of RN outpatient, behavioral health, psychiatric, primary care or community health
  • Comfortable working both in clinic and in community/home settings
  • Strong communication and interpersonal skills
  • Ability to work collaboratively within a multidisciplinary behavioral health team
  • Self-directed and able to manage responsibilities with minimal oversight
  • Ability to work with adults diagnosed with severe mental illnesses
Preferred
  • Experience in behavioral health or case management 
  • Experience conducting home visits or community-based care
  • Familiarity with medication management and patient education 
  • Experience working with high-risk or aging populations

Required Abilities/Skills
  • Strong clinical judgment and ability to assess patients independently in non-clinical settings
  • Ability to make informed decisions and escalate appropriately in community environments
  • Comfortable navigating complex patient needs, including behavioral health and chronic conditions
  • Strong boundary-setting and professional communication skills with vulnerable populations
  • Ability to adapt to unpredictable environments during home visits
Responsibilities/Essential Functions
  • Support MD telehealth visits in clinic, including patient assessment, vitals, and clinical support
  • Perform clinical assessments and monitor patient status during both clinic visits and home visits
  • Conduct toxicology screenings in clinic and during community/home visits
  • Lead medication management activities, including coordinating with pharmacies, assessing adherence, and providing patient education
  • Deliver medications to clients and assess proper use, adherence, and potential concerns
  • Coordinate care with primary care providers regarding chronic conditions and follow-up after ED or hospital visits
  • Conduct home visits to assess patient health, safety, and environmental conditions
  • Provide wound assessment and basic wound care management during home visits
  • Support patients during public health concerns (e.g., lice, scabies, infestations) with appropriate clinical guidance
  • Document all patient interactions, assessments, and care plans accurately and thoroughly
  • Identify and escalate clinical concerns, changes in condition, or safety risks to providers and care team
  • Build therapeutic relationships with patients to support engagement, stability, and continuity of care
