MINMUM QUALIFICATIONS
Education Qualifications
  • Bachelor’s Degree or higher in Nursing from an accredited academic institution.
Licenses and Certifications
  • RN - Registered Nurse
  • BLS - Basic Life Support
Experience
  • 2 years experience as registered nurse with a preference for candidates who have prior clinical experience in Rheumatology
  • Epic in basket experience
  • Pediatric Ambulatory Experience
  • Triage Experience
  • One or more years experience in ambulatory care preferred
Responsibilities

  • Collaborate closely with Rheumatologists to support clinic operations, including assisting with patient follow-ups, triage, and care planning.

  • Perform comprehensive RN-level care coordination including prior authorizations, referrals, follow-ups, and communication with multidisciplinary care teams.

  • Triage patient messages and phone calls using Epic InBasket and escalate clinical concerns appropriately with thorough RN assessments.

  • Provide patient education, resources, and guidance for disease management, medication use, and treatment expectations.

  • Coordinate and follow up on genetic testing, results review, and provider-patient communication regarding next steps.

  • Actively participate in team-based care models to improve clinical efficiency and enhance patient outcomes.

  • Assist with navigating complex care plans and supporting patients through transitions of care or specialty referrals.
KNOWLEDGE/SKILLS & ABILITIES
  • Knowledge in specialty area of practice
  • Knowledge of computer systems and software used in assigned work area
