MINMUM QUALIFICATIONS Education Qualifications
Bachelor’s Degree or higher in Nursing from an accredited academic institution.
Licenses and Certifications
RN - Registered Nurse
BLS - Basic Life Support
Experience
2 years experience as registered nurse with a preference for candidates who have prior clinical experience in Rheumatology
Epic in basket experience
Pediatric Ambulatory Experience
Triage Experience
One or more years experience in ambulatory care preferred
Responsibilities
Collaborate closely with Rheumatologists to support clinic operations, including assisting with patient follow-ups, triage, and care planning.
Perform comprehensive RN-level care coordination including prior authorizations, referrals, follow-ups, and communication with multidisciplinary care teams.
Triage patient messages and phone calls using Epic InBasket and escalate clinical concerns appropriately with thorough RN assessments.
Provide patient education, resources, and guidance for disease management, medication use, and treatment expectations.
Coordinate and follow up on genetic testing, results review, and provider-patient communication regarding next steps.
Actively participate in team-based care models to improve clinical efficiency and enhance patient outcomes.
Assist with navigating complex care plans and supporting patients through transitions of care or specialty referrals.
