MINMUM QUALIFICATIONS Education Qualifications:
High School Diploma or equivalent
Experience:
Two years experience of progressively responsible and directly related work experience (Transfer Center experience for a hospital setting, Emergency Medical Dispatcher, ED Technician)
Preferred:
Paramedic license NREMT/Paramedic or EMT license NREMT preferred
EPIC experience
BLS - Basic Life Support
Skills Required:
Knowledge of Medical Terminology
Knowledge of the Emergency Medical Treatment and Active Labor Act (EMTALA) and related regulations.
Knowledge of the requirements and regulations of third-party payers, such as Medi-Cal, Medicare, CCS and other government programs as well as private insurance preferred.
Ability to perform complex tasks, prioritize multiple projects, and work under pressure to meet deadlines in a fast-paced environment
Ability to remain calm under pressure and apply sound judgment
Responsibilities:
Performs timely and accurate initial telephone intake for pediatric and maternal transfer and consult requests from referring hospitals, clinics and other healthcare facilities
Validates patient identity information and determines whether the patient has an existing medical record before creating a new patient medical record
Continuously triages incoming and pending transfer and consult requests based on available clinical information, patient condition, level of urgency, and current Transfer Center activity
Identifies the appropriate accepting or consulting provider based on clinical information and obtains additional details from the referring clinical team when needed
Coordinates requests by facilitating timely connection between referring providers, accepting or consulting providers, transport teams and other clinical stakeholders
Applies established transfer criteria to determine steps for EMTALA-related requests, higher-level-of-care requests, family-requested transfers, same level-of-care transfers, and requests requiring financial clearance
Maintains shared situational awareness with Transfer Center colleagues by communicating active calls, pending requests, provider connections and relevant updates
Utilizes Transfer Center telephone systems, electronic medical record (EPIC), secure paging system (SmartPage), internal messaging systems (Voalte, Teams chat, etc.) and other electronic tools to support accurate documentation, communication and coordination of requests.
Collaborates with Patient Placement to confirm bed availability, communicate transfer acceptance and provide estimated time of arrival for incoming transfers
Serves as communication point between referring facilities, receiving teams, Patient Placement, and transport partners to support continuity throughout transfer and consultation workflows
Escalates barriers, delays, unclear requests, capacity concerns or workflow deviations according to established Transfer Center escalation pathways
Provides clear handoff of active and pending requests, unresolved barriers and required follow-up actions to Transfer Center colleagues at shift change or during coverage transitions
Maintains professional, clear and service-oriented communication with referring facilities, providers, transport teams, and internal team members to support timely coordination of transfer, consult and transport requests
